Log into your admin area of your cart and go to Cart Settings >> Payment, Shipping & Taxes > Shipping Management >> Custom Methods tab. Click on Add a New Custom Shipping Method and then fill in all the fields with the name of the shipping method, the calculation method which in this case would be 'Price Based', and select the countries and states you want this shipping method to work for.
Fill in the amounts where appropriate, such as shown below if you were going to offer free shipping for any orders over $200 dollars with a max of 9999 dollars qualifying for it.
